Appointments are available from 8:00 am to 7.00pm, Monday through Friday.

First Appointment
Arrive 5-10 minutes early to fill out forms.
- Bring a pair of shorts for lower extremity injury and back injuries.
- Bring a sleeveless t-shirt or tank top for an upper extremity injury.
- Bring your doctor's referral slip if you have one.
- Bring your Health Insurance Details if appropriate.
- Be prepared for your appointment to last 45 minutes.
